Effect of late sodium current inhibition on MRI measured diastolic dysfunction and myocardial perfusion reserve in aortic stenosis: a pilot study
نویسندگان
چکیده
Background Aortic Stenosis (AS) is characterized by pressure overload hypertrophy (LVH) with associated diastolic dysfunction. The presence of LVH, diastolic dysfunction and microvascular dysfunction appear to be important determinants of exercise capacity in AS. There are no medical therapies in AS of proven value. Ranolazine is licenced for the treatment of angina and inhibits late sodium channel activation. Ranolazine has been shown to improve diastolic dysfunction in isolated myocytes.

A randomized, placebo-controlled trial of late Na current inhibition (ranolazine) in coronary microvascular dysfunction (CMD): impact on angina and myocardial perfusion reserve
AIMS The mechanistic basis of the symptoms and signs of myocardial ischaemia in patients without obstructive coronary artery disease (CAD) and evidence of coronary microvascular dysfunction (CMD) is unclear. The aim of this study was to mechanistically test short-term late sodium current inhibition (ranolazine) in such subjects on angina, myocardial perfusion reserve index, and diastolic fillin...
متن کاملResolution of Angina Pectoris and Improvement of the Coronary Flow Reserve after Ranolazine Treatment in a Woman with Isolated Impaired Coronary Microcirculation
In a 61-year-old woman with well controlled arterial hypertension, hypercholesterolemia, and smoke and suffering from recurrent angina pectoris despite angiographically normal epicardial coronary vessels and maximal therapy, the replacement of nitrates with novel antiangina drug ranolazine, after 6-month therapy, induced a complete relief of angina and a relevant rising of the transthoracic Dop...
متن کاملThe role of oral dipyridamole stress gated SPECT in assessing the response of the left ventricle to stress: Re-evaluation of an old method
Introduction: We aimed to assess whether the vasodilator effect of oral dipyridamole on the left ventricular systolic function in patients with suspected CAD is different from that of intravenous (IV) dipyridamole using Tc-99m MIBI myocardial perfusion gated SPECT. Methods: Eighty-nine patients (17 M / 72 W; 61 ± 10 years) were enrolled in this study. The patients underwent a dipyridamole stre...
متن کاملSevere aortic stenosis has blunted myocardial T1 relaxation response to vasodilator stress: a cardiac magnetic resonance adenosine stress test study
Background Aortic stenosis is characterized by impaired myocardial perfusion reserve due to coronary microvascular dysfunction. T1 mapping is sensitive to myocardial water content of both intraand extracellular in origin, but the effect of intravascular compartment changes on T1 has never been assessed previously. We aimed to assess the effect of adenosine-induced vasodilatation on native (pre-...
متن کاملNitric oxide's role in the heart: control of beating or breathing?
Beneficial actions of nitric oxide (NO) in failing myocardium have frequently been overshadowed by poorly documented negative inotropic effects mainly derived from in vitro cardiac preparations. NO's beneficial actions include control of myocardial energetics and improvement of left ventricular (LV) diastolic distensibility. In isolated cardiomyocytes, administration of NO increases their diast...
